2. Once you fill the form, you will get a PDF file on your email. This file will have 2 pages.
3. Take 2 physical prints of the file. Submit one print of page 1 to Surgery Clerk office. Keep the second print of page 1 with you. Bring both prints of page 2 at the time of DDRC meeting.
